ABSTRACT

Thrombocytopenia is a common extrahepatic manifestation of chronic viral hepatitis C. Many mechanisms are implicated in its genesis, specially the immunological factor. Several ways of treatment were used with a temporal efficiency. We reported an observation of a 62 year old female-pateint of chronic viral hepatitis C, who developed a severe thrombocytopenic purpura. Steroid therapy was ineffective. Treatment of such cases is difficult as association of immunosuppressive therapy increases the platelet count, but lad to severe hepatocellular insufficiency due to its negative effect on the hepatic function. Using other molecules as the immunoglobulines or interleukin II constitute a promising alternative way of treatment

ABSTRACT

Upper Gastrointestinal haemorrhages of ulcerous origin onstitutea frequent and fearsome emerrgency. We report the experience of our department about the epidemiological aspects of peptic haemorrhagic ulcers. Our study was carried out during the period of two years [January 1998 - december 1999] in the endoscopy unit of our ward and conducted over 600 patients presenting with an acute upper gastrointestinal heamorrhage. Only upper gastrointestinal heamorrhage of ulcerous origin will be studied. The mean age of our patients is 48 years with a male predominance [88,9%]. Antecedents of peptic ulcer and acute upper gastrointestinal heamorrhage were noted respectively in 17% and 6,8%. An associated disease was noted 29,7% and a consumption of a gastrotoxic drug in 6.5%. On the 600 upper endoscopy realized, a peptic ulcer was the cause of the heamorrhage in 235 cases [39,2%]. The ulcer was duodenal in 79,1%, gastric in 15,4%, pyloric in 1.7%. The double association duodenal and gastric was noted in 3,8%. The front face of the bulb was the most frequent localisation of the heamorrhage [74,1%]. A lesional association, heamorrhagic or not, was found respectively in 36,5% and 38,9% with duodenal ulcer and gastric ulcer

ABSTRACT

A retrospective study was done in our Departement "C" of the evolutive characteristic of gastric ulcers in 103 patients. The frequency of gastric ulcers in our endoscopic unit is 1,84%, the sex ratio is 3,5; 67% of patients concern age group between 40 - 60 years. In 40,7% cases the inaugural symptoms were typical, while 33,9% atypical and whereas 25,2% presented; with bleeding. The small curvature is by far the selective position of gastric ulcers whereas the large curvature in our series is only 5,85%. Medical treatment is efficient in 80,5% of cases with cicatrisation confirmed by endoscopy and only 19,7% of our patients necessitated surgical intervention. Out of 72 cases seen regularly with endoscopic controls and multiple biopsies, there was one only case of neoplasy confirmed histologically at 5th. control with biopsy; showing the importance of surveillance endoscopic together with biopsy

ABSTRACT

Our study concerns 12 patients with Biermer disease in department C of the university hospital in Rabat, between the period 1977 - 1983. These patients were admitted mostly for anemia with a frequency of 0.18% of hospitalisation for This period noted. The average age for this disease was 48 years with masculine predominance of 8 to 4. We observed that this disease is exceptionally rare in children and must ceased this opportunity to emphasize the clinical, paraclinical and evolutive characteristic of this affection
